This is to certify Kidney Dialysis patient Rakhi Sharad Lamture, 29/F, years was Treatment Kokilaben Dhirubahi Ambani Hospital & Medical Research Andheri (W)..Mrs.Rakhi Lamture, a female patient to kidney dialysis department with above mentioned complaints. She was breathless tachycardic and tachypneic.
This is to inform you that Mrs. Rakhi Lamture, is a known case of End Stage Kidney Disease on maintainance hermodialysis thrice a week
Her monthly expenses are as follows
1) A session of hemodialysis : Rs 2000
2)Dialyser and tubing to be changed after 10 session of hemodialysis Rs.2000 Every 10 days
3) Every week 3 time Disease a month (Rs.24000 a month)
