Had a meeting with my oncologist and nephrologist today to discuss the roadmap for Myeloma and Kidney Transplant. The 2 important points of discussion were

1. After how much time of SCT could a kidney transplant be done - As per the doctors, mine will be the first case of it's type in which they will perform kidney transplant for a Myeloma patient who underwent SCT, in whole of India and probably a few handful of such cases around the world. So they don't have any prior data at their convenience. What they are going to do is to send my case history to Mayo clinic USA which is probably the leader in treating cancer and renal problems. They are hoping for a response from them and then proceed further.

2. Risk factor - In case they do kidney transplant and Myeloma comes back, there are chances that it will try and target the transplanted kidney as well.

Minimum time frame on which Myeloma pateints are kept for maintenance is 2 years. Chances of relapse reduce considerably after 2 years. So should we wait minumun of 2 years before going for kidney transplant ? There are complications related to the immunosupperants that I will need to take after kidney transplant and the maintenance drugs of Myeloma. Both medications contradict each other. So there is a question mark on how to go about the business here if kidney transplant needs to be performed before 2 years of SCT. Also the problems of performing dialysis twice or thrice a week for 2 years is a big ask.

These were some of the discussion points. Anyways my oncologist has asked me to undertake another Myeloma Panel test, the results of which they will share with Mayo clinic USA along with my case history.

03/04/3018 update: Update : Went to meet my oncologist yesterday. For my case that he presented to Mayo Clinic USA, they responded. They feel that 9 months to 1 year is ideal time frame after SCT to go for kidney transplant. They think that Myeloma might come back in future and in case it does the transplanted kidney can be protected with medicines. Oncologist did tell us to meet our Nephrologist and proceed with the transplant process. So we met our Nephrologist today. He is called Mom (donor) on Tuesday and set a time frame of arnd Aug for the transplant.

Hello Everyone,  

Wishing u all very happy Gurupurab.  
It is day 112th for Amar's post sct and all the necessary tests(PET Scan and Myeloma panel) that were supposed to be done post 100th day of Sct are complete and reports show no trace of myeloma. He is on maintenance drugs.  On the kidneys front,  there has been no improvement and he is still on dialysis twice a week.  

He has 2 choices,  either to continue dialysis and see if any improvement happens in due course of time or go for kidney transplant which can happen only after at least 6 months of Sct and making sure he is still in remission ( cancer free).  

Meanwhile, he is gaining strength day by day and is doing physical activities like early morning walk and yoga pranayamas. These exercises are helping his backbone in gaining strength but it is not to its full potential yet.
