dear sir,             my name is A V SRINIVAS I am staying in mysore ,rented with family of two children.I am working as a sales executive at water purifier company.
last year in 17/5/2018 I meet accident in front of suburb bustand , frm the  back side car came&hit my two wheeler I fall down  havey  force frm my two wheeler& at that time my clavcal left side bone broken.after that I went to government hospital for treatment&doctor told  your clavical bone broken and advice me to surgery to be done.then I came to kamakshi hospital&admited for surgery,on 19/5/2018 my surgery done by Dr nagabushan specialist in ortho surgery.opreation is success.my bill amount is 35000/-paid by orental bank of commerce through oriental insurance.this time I am not renew my health insurance due to financial trouble.
doctor advice me to admit in the month of December 2019 may be on 20/12/2019
at present I am working at shri vijaylakshmi kitchen appliances,mysore as a counter sales executive&my take home salary is 10000/(-ten thousand only)kindly give fincial support at the time of opreation
