This is to inform you that Mr. Vijay Mandal was admitted on 18/09/2019 at 12.00AM in ICU ward with allowed H/O RTO hit by a lorry while traveling in two wheeler near PES ENG college E.city Bangalore at around 1/PM on 17/09/2019 with H/O Head injury severe bleeding Patient brought to causality in altered sensorium, injury to the left hand, cervical injury, nasal bleeding,injury to the left leg, hematoma in neck, hemorrhagic shock , hemo dynamically unstable.
Patient required ventilator support, fluids recomandation with blood and other product’s .
Neuron Surgeon, Orthopedic Surgeon, General physician, Anesthesia opinion sought , patient, intubated and put on mechanical ventilator with inotropes support,with continued physiotherapy.

PERSONAL HISTORY :
Patient basically from Bihar state,India ,was came for to work as a labour to Banglore 3 months back.he is an elder one in his family to manage his family member by financialy. And had lives below poverty level, at present his family members financial status his very poor and he is not able to manage daily cost of living and even he is not afforadable to get daily medicines.
And he requires funds to continuing treatment, physiotherapy and next level of Rehabilitation and to take care his family members and leading life and in future he has a wife and 3 children, mother and 4 younger brothers. He is the only one person to lead the whole family, and kindly help to him and his family members to come out of this incidence.
