Mr. K Kishan Naik, aged 46 years/ male, is suffering from (Diagnosis):- “ B/L PYELONEPHRITIS, BALANOPROSTHITUIS, IRON DEFECIENCY, PENILE ULCERATION, HBS AG + VE ”, Treated with urine output of 500ml , pain in supra pubic region relieved . All the necessary investigations were done. Blood Parameters shown of elevated tlc , inflammatory makers . Hbsag is positive . Ct abdomen suggestive of b/l perinephric fat stranding . In view of findings suggestive of urosepsis, urologist opinion taken and followed accordingly, antibiotics escalated, cultures sent, suggestive of no growth. In view of penile ulceration plastic surgeon opinion taken advised interval debridement+/- grafting  and iv antibiotics, iv antipyretics, iv antiemetics,iv fluids, iv PPIs and other supportive care
