Dear sir/madam
This is regarding ms.maheen kousar (hospital no.4171399/1361731)4 ¹/² year old female child.who as been diagnosed as end stage renal disease(ESRD)we have counselled the family the need of renal replacement therapy as life saving measure at present child is undergoing acute treatment in PICU.now on emergency basis child is on haemodialysis.she required CAPD(continuous ambulatory peritoneal dialysis)as long term treatment plan and later renal transplant.her approximate cost of present admission will be Rs.1,50,000 -2,00,000/-
Future approximate cost towards dialysis, investigation and medicines are Rs.30000/- per month.the approximate cost of renal transplantation surgery is Rs.6,00,000/-
The family belongs to low economical status.they are finding it difficult to continue the treatment due to financial constraints.kindly consider this child for financial assistance.
All contribution can be issued in favour of "st.john's medical college hospital" with a letter of intent mentioning the name and hospital number of the child
